Care To Independence

The “Launch” transition from Care to Independence service offers mobile, community based support to young people transitioning from care to independent living, across the Auckland area. This service is youth-led and provides support and advice to young people aged 15-20 years who have a goal of independent living.


A Personal Advisor provides one-on-one, individual support to each young person to help them to achieve their goals, linking them to other services and resources where required and ensuring they have access to all that is available to them.

The primary referral pathway requires the Oranga Tamariki Social Worker to complete the ‘Request for Joint Assessment Form’ and submit it to the “Transition from Care to Independence” transition group at the Regional Oranga Tamariki office. This will provide for an NGO Personal Advisor to complete an assessment of the young person’s circumstances, particularly in relation to their transitional needs. This assessment will recommend their suitability for acceptance to the Launch programme, or the equivalent service offered by Youth Horizons Trust called ‘Ka Awatea’.
